XERF or Morpheus8:
Which Is Right for You?

Radiofrequency treatments can improve skin laxity, facial contours, fine lines, and uneven texture—but not all RF treatments work in the same way.

XERF and Morpheus8 both use radiofrequency energy to heat targeted tissue and stimulate remodeling. However, they differ significantly in how the energy is delivered, the concerns they are best suited to address, the level of discomfort, and the expected downtime.

Understanding these differences is important when choosing the right treatment for your skin.

What Is XERF?

XERF is a non-invasive monopolar radiofrequency treatment designed to improve skin firmness and facial contours without needles.

The system combines two radiofrequency frequencies—6.78 MHz and 2 MHz—to deliver energy at different tissue depths. It also offers multiple depth and intensity settings, allowing the treatment to be adjusted according to each patient's skin condition and facial anatomy.

XERF may be suitable if your main concerns include:

  • Facial sagging or loss of firmness
  • Reduced jawline definition
  • Mild to moderate skin laxity
  • A preference for a comfortable, non-invasive treatment
  • Minimal downtime
  • Avoiding needles or numbing treatment

During treatment, patients usually experience a sensation of warmth. Because the skin surface is protected by an integrated cooling system, treatment can generally be performed without topical anesthesia.

Most patients can return to their normal activities immediately afterward, although temporary redness or warmth may occur.

What Is Morpheus8?

Morpheus8 is a fractional radiofrequency microneedling treatment. It uses small needles to penetrate the skin and deliver bipolar RF energy at selected depths.

This combination creates controlled micro-injuries while heating targeted tissue. It can therefore address skin texture as well as laxity and deeper tissue remodeling.

Morpheus8 may be considered if your main concerns include:

  • Acne scars
  • Enlarged pores
  • Uneven skin texture
  • Fine lines
  • Skin laxity
  • More intensive dermal or subdermal remodeling

Because Morpheus8 uses microneedles, topical or local anesthesia may be required depending on the treatment area and settings. Temporary redness, swelling, sensitivity, pinpoint bleeding, or small crusts may occur. Downtime varies, but patients should generally expect several days of visible recovery.

The Most Important Difference: Non-Invasive RF vs RF Microneedling

The main difference is the way radiofrequency energy enters the tissue.

XERF delivers energy through the skin's surface without needles. It is primarily selected when the goal is comfortable, non-invasive tightening with little interruption to daily life.

Morpheus8 uses needles to deliver RF energy directly at predetermined depths. This makes it particularly useful when the treatment goal includes texture, acne scarring, pores, or more intensive remodeling.

Neither treatment is universally “better.” The right option depends on what we are trying to improve and the condition of the individual patient's skin and underlying facial tissue.

Can Morpheus8 Affect Facial Fat?

Depending on the needle depth, energy level, number of passes, treatment area, and individual anatomy, Morpheus8 may deliver energy into or close to the subcutaneous fat layer.

In selected patients and areas, deeper tissue remodeling may be part of the intended treatment plan. However, facial fat plays an important role in maintaining youthful contours. Excessive or unintended volume loss may make the face appear more hollow or aged.

In October 2025, the U.S. FDA issued a safety communication regarding reported complications associated with certain uses of RF microneedling devices. These reports included burns, scarring, fat loss, nerve damage, and other injuries. The FDA emphasized that RF microneedling is a medical procedure that should be performed by appropriately trained and licensed healthcare professionals. Read the FDA safety communication.

This does not mean that every patient will experience these complications. It does mean that device selection, treatment depth, energy, technique, and patient assessment are extremely important.

At our clinic, we carefully evaluate:

  • Facial anatomy and proportions
  • Skin thickness and quality
  • The location and amount of facial fat
  • Areas of existing volume loss
  • The degree of skin laxity
  • Acne scarring and texture
  • Previous energy-based treatments
  • The patient's goals and tolerance for downtime

Morpheus8 settings should be customized not only for each patient, but also for each area of the face. A single depth or energy level is not appropriate for every treatment zone.

Which Treatment Has Less Downtime?

XERF generally has less downtime because it does not puncture the skin. It may therefore be preferable for patients who have busy schedules, want to avoid needles, or cannot accommodate several days of recovery.

Morpheus8 involves microneedling and more intensive tissue remodeling. Although recovery varies according to the settings and individual skin response, patients should expect more redness and visible downtime than with XERF.

Can XERF and Morpheus8 Be Combined?

In some cases, the treatments may complement each other because they address different concerns. XERF may be used to support non-invasive tightening and contouring, while Morpheus8 may be selected for targeted improvement of texture, scars, pores, or deeper laxity.

However, combining treatments is not automatically better. The need for both procedures—and the appropriate interval between them—should be determined after a medical assessment. Treatment should be based on the patient's anatomy and priorities rather than applying the same protocol to everyone.

In Simple Terms

Choose XERF if you want comfortable, non-invasive skin tightening, improved firmness, better jawline definition, and minimal downtime.

Consider Morpheus8 if your priorities include acne scars, enlarged pores, uneven texture, fine lines, or more intensive tissue remodeling—and you are comfortable with needles and several days of recovery.

The best treatment is not simply the strongest or deepest option. It is the treatment that delivers the appropriate amount of energy to the appropriate tissue layer while preserving the natural structure and balance of your face.
